Colonial Poplin and Poplin Way offer assisted living, skilled nursing, rehabilitation, and long term care services – all under one roof. We are proud to be a privately owned and operated establishment, providing a comfortable and home-like environment for residents, patients, and visitors. We offer the senior community three main services, through our community assisted living residence and nursing home and rehabilitation center. At both Colonial Poplin and Poplin Way we believe in the person-centered approach to care, always treating our residents with dignity and respect, ensuring their total health needs are met, and they achieve their highest levels of functional independence.
Poplin Way is a socially active Assisted Living Community. Our privately owned assisted living community offers around the clock nursing assistance in a charming, country environment. Residents live in their own apartment, with that added benefit of having convenient access to skilled nursing care in the same location, when needed.
Our 50-bed Medicare and Medicaid Certified skilled nursing/rehabilitation home provides individualized services to meet the diverse cognitive and physical abilities of our residents. Short Term Skilled Nursing and Rehabilitation services are offered at the Colonial Poplin nursing facility, where patients receive daily treatment to achieve recovery and strength to avoid re-hospitalization after they’ve been discharged home. Colonial Poplin residents live in a socially active senior living community that feels like home, while also receiving daily, individualized long-term skilled nursing and rehabilitation services from a friendly and caring staff.
For the 18th year in a row our community has earned the status of “deficiency free”, which indicates that we not only meets the state’s safety and quality standards, but that its care is exemplary and without need for any improvement. To earn this status, senior living communities are evaluated by state officials following an extensive, unannounced survey, which is generally conducted on site. Inspectors judge a senior living community’s performance on safety measures, quality of care, medication management, hygiene, food service, and much more. We are very proud of this achievement, and are committed to continually earning this status.
