Welcome Home.
Our family, along with our highly compassionate staff, offers assisted living services and a strong continuum of skilled nursing care – all in one location. Russell and Karen Philbrick established the community in 1983 with an uncompromised understanding of the need for a comfortable and caring atmosphere.
At Colonial Poplin and Poplin Way, We Believe:
Colonial Poplin, a 50-bed community skilled nursing center, is both Medicare and Medicaid-certified. Our skilled nursing home has earned the reputation of being one of the Country’s finest skilled rehabilitation and long-term nursing care communities.
Situated on 12.5 tree lined acres in the picturesque town of Fremont, Poplin Way Assisted Living is adorned with New England inspired paintings and charming décor throughout, creating the ambiance of a country inn. As a “continuum of care campus,” our skilled nursing care is just next door, if needed.
Through our ongoing quality improvement process, we continuously review our practices and standards preserving the integrity of our family owned and operated business. Our reputation throughout New England is unparalleled with clinical surveys that have always been deficiency free.
Our services are individualized to meet diverse cognitive and physical abilities, and our effectiveness is largely due to the smaller size of our community and and commitment of our caring staff. Meet our dedicated management staff.
We hope you’ll consider Poplin Way and Colonial Poplin to provide the assisted living, skilled nursing, and/or rehabilitation services you need.
